Abstract

The study aimed at investigating the generation, organization and use of medical records in Primary Health Care Centres of Ahmadu Bello University, Zaria. To achieve these objectives, six research questions were formulated and answered accordingly.

Review of related literature was also carried out which covered types of medical records generated when they are generated and the use of these medical records in primary health care centres. The survey research method was adopted for this study.

The population under this study consisted of record staff, nurses, laboratory technicians, and doctors of all the Primary Health Care Centres in Ahmadu Bello University Zaria. A total of 100 respondents from the population formed the sample for this study.

The five primary Health Care centres studied were University Health Care Services Samaru-Main Campus, University Health Care Services Kongo Campus, National Animal Production Institute (NAPRI) Clinic Shika, Institute of Agricultural Research (IAR) Clinic, Samaru and Divisional of Agricultural Colleges (DAC) Clinic, Samaru.

The data was collected by distributing questionnaires, which was the instrument used for data collection. The data collected were analyzed using the frequency distribution tables, simple percentages and histograms.

1.1 Background of the Study

In every human endeavour, records are generated on a daily basis. These records serve as a channel of expression for most organizations, institutions or agencies.

For organisations to achieve their set-up goals and objectives in their day to day operations, information must be well captured at the point of generation and the ability to organize them well will determine their usability and accessibility.

They are not only generated and received, but they constitute the pillars that keep an organization in all aspects of its operations and existence almost on daily basis.

These records contain information which is crucial to human endeavour as reported by Hassan (2009), that information is an indispensable tool in office work, management, and decision making and in work productivity.

This is to say that effectively organized and good management of private and public sectors/organizations‟ records, depend heavily on the availability of current, complete, accurate and reliable information which is generated and supplied on time to facilitate planning, decision making and in order to enhance productivity.
